Our company used the Consumer Deks in a study of 276 diabetics with HgBA1'c greater than 10. We sent
every other patient a Dek along with a phone number to call for follow upquestions.
We followed their A1C's for a year afterward and realized a drop on average of 3 points in A1C
results. The results speak for themselves. The Deks are an immensely helpful tool in good diabetes
management and a "must have" for PCP's to manage diabetic population.

Paul Davidson MD, FACE
One of the nation's original board-recognized endocrinologists, has worked with diabetes for 42 years. Diabetes experts in Minnesota, West Virginia, University of Pennsylvania, and Emory trained him, and he has in turn trained others at West Virginia University and Emory. He has experience in bench and clinical research, medical editing and writing, establishing, administrating, and teaching recurring seminars to help persons with diabetes maintain themselves in safe blood glucose target ranges. His research on insulin and triglyceride measurement in 1965 was the basis for the now widely recognized insulin resistance syndrome. Because of his involvement in a large 1960-1970's research study (UGDP); he became an early advocate of intensive patient education and aggressive management of diabetes for tight blood glucose control. He developed innovative management systems, which have been published and are now used worldwide. One such protocol was developed for Glucommander, a computerized system for IV insulin administration, developed with Dr. Dennis Steed. He is the founder and senior member of the 20 year old and largest diabetes practice in Southeastern United States, Atlanta Diabetes Associates. He is the medical director with the longest tenure of the nation-wide, 100 site Diabetes Treatment Centers of America. He is one of ten physicians in the US who has expertise in the use of the implanted insulin pump. In the past few years he has collected over 2200 records of insulin pump management of diabetes and from this he has evolved a new system of normal blood glucose diabetes management, patented as PumpMaster.
Bruce Bode, MD, FACE
A diabetes specialist with Atlanta Diabetes Associates with special expertise and interest in insulin delivery and glucose sensing. He is past president of the Georgia Affiliate of the American Diabetes Association (ADA) and has served on the board of directors of the Atlanta chapters of the Juvenile Diabetes Foundation, the ADA, and Georgia Diabetes camps. Dr. Bode received his doctorate of medicine from Emory University School of Medicine and his bachelor's degree in biology from the College of Wooster in Wooster, Ohio. He completed a fellowship in diabetes with Paul C. Davidson, MD, and an internship and residency at Emory University Affiliated Hospitals. Dr. Bode has co written or contributed to several books, including ADA's Management of Type 1 Diabetes, 4th edition, Diabetes DEK How to Control and Manage Diabetes Mellitus and The Insulin Pump Therapy Book: Insights from the Experts. He is a member of the medical advisory boards of Diabetes Interview, Medtronic Minimed, Novo Nordisk Pharmaceuticals, sanofi-aventis Pharmaceuticals, and DexCom. In addition, Dr. Bode has written or co written over 100 articles and abstracts for professional journals such as Diabetes, Diabetes Care, The Journal of New Developments in Clinical Medicine, Diabetes Metabolism Research and Reviews, Diabetes Spectrum, and Postgraduate Medicine. He is also an editor of the Consumer DiabetesDek. Dr. Bode's goal in life is to help people with diabetes better manage their disease with prevention of not only the complications of this disease but an attainment of a normal quality of life.
David Robertson MD, FACE
Currently practices Endocrinology with the Atlanta Diabetes Associates located at Piedmont Hospital. He has been with this practice since 1998. Previously he was in practice at the Emory Clinic for six years. In addition to general Endocrinology, his practice focuses on metabolic risks for cardiovascular disease and endocrine disease in transplant patients. Previously, he was part of the faculty at Emory University School of Medicine. His faculty association was with both the Division of Endocrinology and the Division of Arteriosclerosis and Lipid Metabolism. Currently, he is also associated with the Fuqua Heart Center at Piedmont Hospital as an investigator focusing on diabetes and cardiovascular disease. Prior to his association with Piedmont Hospital, he was a National Institute of Health investigator at Emory University who received a Physician Scientist Award focused on the regulation of gene expression by insulin in primary hepatocyte and adipocyte cultures. His current investigations at Piedmont Hospital continue his research support through the National Institute of Health. He is the diabetes principal investigator for the BARI 2D (Bypass Angioplasty Revascularization Investigation in Type 2 Diabetes) trial. The trial hopes to define optimal strategies for the management of active coronary artery disease in the patients with Type 2 Diabetes. Within his practice he is involved in numerous clinical trials related to the treatment of both type 1 and type 2 diabetes, the treatment of combined dyslipidemia, the treatment of hypercholesterolemia, and treatments to improve insulin sensitivity and reduce cardiovascular risks. He has published papers in all of these areas.
N. Spencer Welch, MD, FACE
Born and raised in Atlanta. He received his undergraduate degree at Princeton University and then came home to Emory University for Medical School. He did his internship and residency at Emory, Grady Hospital and affiliated hospitals. He did a chief residency at Crawford Long Hospital before doing a Fellowship in Endocrinology at Emory where he did research in developing a method to measure insulin resistance in Type 2 Diabetes patients. Since fellowship Dr. Welch has practiced for 18 years at Piedmont Hospital, helping them develop their Diabetes Resource Center, which helps all people with Diabetes to live better. He takes a multi-disciplinary approach to caring for Diabetes which focuses not only on getting a patients blood sugar, blood pressure, and cholesterol down, but also focuses on getting patients to develop proper lifestyles to help them live better and healthier lives. In addition to caring for people with diabetes Dr. Welch practices general adult endocrinology and has a large patient population with Thyroid disorders. He also treats patients with metabolic bone disease such as osteoporosis and patients with hormone deficiencies and/or excesses resulting from pituitary or adrenal disorders. Lastly he sees men with problems relating to testosterone deficiency, which causes loss of sex drive and or sexual functions.
Jay S. Skyler, M.D., MACP
Currently a Professor of Medicine, Pediatrics, & Psychology, in the Division of Endocrinology, Diabetes, & Metabolism, Department of Medicine, University of Miami Miller School of Medicine, Miami, Florida. He served as Director of that Division from 2000 to 2004. At the University of Miami, he is Associate Director for Academic Programs in the Diabetes Research Institute. He is Chairman of the NIH (NIDDK)-sponsored Type 1 Diabetes TrialNet, an international network conducting clinical trials to prevent type 1 diabetes or interdict the type 1 diabetes disease process. He is a past President of the American Diabetes Association, the International Diabetes Immunotherapy Group, and the Southern Society for Clinical Investigation, and was a Vice-President of the International Diabetes Federation. He served as a member of the Endocrinology, Diabetes, and Metabolism Subspecialty Examining Board of the American Board of Internal Medicine, as Chairman of the Council of Subspecialty Societies of the American College of Physicians (ACP) and a member of the ACP Board of Regents. He was founding Editor-in-Chief of Diabetes Care, and currently is Scientific Editor of International Diabetes Monitor and Associate Editor of Diabetes Technology & Therapeutics. Dr. Skyler's research interests are in clinical aspects of diabetes, particularly improving the care of type 1 diabetes through meticulous glycemic control, psychosocial and behavioral support, and immune intervention. He is widely acclaimed for developing 'algorithms' for patient adjustment of insulin doses.

Anne Daly, MS, RD, BC-ADM, CDE
A nationally recognized author, consultant, and expert in diabetes care and education, obesity and weight management. A national Past-President, Health Care & Education of the American Diabetes Association, she was a co-founder of the Springfield Diabetes & Endocrine Center in Springfield, IL, the largest diabetes practice between Chicago and St. Louis. As Director of Nutrition and Diabetes Education there, she manages an American Diabetes Association Education Recognized Program and a comprehensive weight management/lifestyle change program. Previously, Anne served as Assistant Professor of Clinical Medicine in the Department of Internal Medicine, Division of Endocrinology, Southern Illinois University School of Medicine. An award-winning dietitian, Anne is the recipient of numerous honors from the American Dietetic Association and its Illinois Affiliate, the American Diabetes Association, and the American Association of Diabetes Educators (AADE). She has received the prestigious Medallion Award from the American Dietetic Association, the Distinguished Service Award from the Diabetes Care and Education Practice Group of the American Dietetic Association, and numerous awards from the American Diabetes Association, including the Rachmeil Levine Award for Distinguished Service, the Outstanding Educator in Diabetes, Outstanding Dietitian of the Year, Outstanding Legislative Activity Award, and the People Recruiting People Award. She is active in district, state, and national dietetic and diabetes associations, and has served in numerous appointed and elected positions. Currently, Anne serves Public Policy Coordinator for the Weight Management Dietetic Practice Group of the American Dietetic Association, a member of the national Nominating Committee of The American Dietetic Association and the Professional Practice Committee of AADE. Anne has published widely in peer-reviewed journals and publications for the public, including 101 Tips for Weight Loss to Prevent and Treat Diabetes and 101 Diabetes & Nutrition Q & A for Health Professionals, both published by the American Diabetes Association. Anne's passion for excellence in diabetes care, prevention of diabetes complications, and advocating for people with diabetes has characterized her 30 year career.
R. Keith Campbell, PharmD, MBA
Is the former associate dean and is the Distinguished Professor of Pharmacy at the Washington State University College of Pharmacy in Pullman, Washington. He has been a member of the faculty since 1968. He was the chair of the Medical Advisory Board for the Washington State Diabetes Control Project funded by the Center for Disease Control. He was a member of professional panels that wrote the position statements on blood glucose monitoring for both ADA and AADE and assisted the NIH in translating the famous Diabetes Control and Complications Trial to try to change the treatment objectives of diabetes patients. He has a personal interest in diabetes having had type 1 diabetes for over 56 years. He has used an insulin pump for over 26 years. Campbell has served on the Board of Directors of the American Diabetes Association and the American Association of Diabetes Educators. He also served on the Board of Trustees of the Diabetes Research and Education Foundation. He has authored over 600 publications and serves on the editorial boards of pharmacy, nursing, medical and lay journals. He serves on the advisory boards of the Diabetes Education Program at Pullman Memorial Hospital and the Spokane Community Health Education Resources. He also has worked with numerous managed- care organizations to develop diabetes treatment protocols. He has been an advisor or board member to the Spokane Chapters of JDF and ADA. Campbell was the first recipient of the O. Charles Olson, M.D. Lectureship. He was the College of Pharmacy's first recipient of the Outstanding Alumnus Award. In 1990, he was presented with the University's President's Faculty Excellence Award for Public Service. In 1988, Campbell was given the special honor of being named The Distinguished Person of the Year for Pharmacists Planning Service, Inc., and in 1997 was presented the national Diabetes Management Leadership Award by the Boehringer Mannheim/Zitter Group. Most notably, he was named the Outstanding Healthcare Educator in the Field of Diabetes in the United States in 1989 by the ADA. The American Association of Diabetes Educators presented him with the Distinguished Service Award. His state pharmacy association named him their 'Pharmacist of the Year' and he received their Don Katterman Distinguished Endeavors Award. He is a fellow of both the American Society of Health System Pharmacists and the American Pharmaceutical Association. Campbell is a Certified Diabetes Educator, and served as a consultant to the FDA's and USP's Endocrine panels. In 2003, Campbell was honored by ACCP with the prestigious Paul F. Parker Medal for his sustained impact on the practice of pharmacy. His University has honored him with the R. Keith Campbell Endowed Chair in Diabetes Care.
Charlotte Hayes, MMSc, MS, RD, CDE
Is a nationally recognized author, consultant, and expert in diabetes care and education, diabetes nutrition and exercise. Charlotte is a Past President of the Greater Atlanta Association of Diabetes Educators, a member of the Diabetes Care and Education Practice Group of the American Dietetic Association and editor of the newsletter On the Cutting Edge, published three times per year. On the Cutting Edge includes peer-reviewed articles and pre-approved continuing education credits. She also chaired the Writing Group for Position Stand on Exercise and Diabetes for the American Association of Diabetes Educators. Her professional affiliations include the American Association of Diabetes Educators, American College of Sports Medicine, American Diabetes Association, and the American Dietetic Association. Q: How ofeten are the Deks updated?
A: At least once a year.
Q: I have never been diagnosed with diabetes. Will the Dek tell me if I have it?
A: Only your healthcare professional can actually diagnose the disease. But, the Dek can help you identify the signs and symptoms of diabetes and will also help you prepare questions to ask and information to relate to your doctor.
A: Part of the good news is diabetes, if caught early and treated properly, can be managed in such a way that most people can live a relatively normal life. This is part of why we are so proud to be able to offer the Diabetes Deks as they have helped many do just that. However, left untreated diabetes can cause blindness, lead to amputations, heart attacks and strokes!
